Membership Questions

No. IV therapy, NAD+ infusions, aesthetics, and skin analysis are open to everyone. No membership required. Members do get a 25% discount on IV therapy.

It means you can receive that service as part of your membership care. The provider visits and management are covered. Medications and labs are purchased separately at wholesale pricing.

Two plans include medication: Men's Optimization ($175/mo) includes testosterone and labs. Metabolic Reset ($499/mo) includes GLP-1 medication. All other plans, medications are separate.

Peptides can be prescribed through the Essential, Signature, and other plans — but the medication is always at cost to you. No membership tier includes peptide medication in the monthly price. If you're interested in peptides, we add IGF-1 to your initial lab panel to make sure it's appropriate for you.

Yes. There are no long-term contracts. You can cancel at any time with 30 days notice.

VitaLife is a Direct Primary Care practice. We don't bill insurance for membership services. This means no copays, no surprise bills, and no insurance company deciding your care. You can still use your insurance for outside referrals, imaging, and hospital visits.

Yes. Courtney sees patients via telemedicine across the entire state of Arizona. Same provider, same quality of care.

Lab panels at VitaLife range from $50 to $300 depending on how extensive they are — typically one-tenth the cost at major lab companies. Our initial evaluation panel (CBC, CMP, lipid profile, complete hormone panel, thyroid, Vitamin D, B12, and more) runs $200-$300. The same panel at a major lab would cost over $2,500.

Insurance, Cost, and Coverage Questions

Membership plans start at $150/month for the Signature plan. The Essential plan is $399/year plus $20/month. Men's Optimization is $175/month. The Metabolic Reset Program is $499/month for 5 months. Family Membership is $350/month.

VitaLife is a Direct Primary Care practice. We don't bill insurance for membership services. This means no copays, no surprise bills, and no insurance company telling us what care you can receive. You keep your insurance for the things it's actually good for: specialist visits, imaging, hospital stays, and emergencies. Most members pair their VitaLife membership with a high-deductible health plan (HDHP), which lowers their monthly insurance premium significantly.

Yes. VitaLife memberships are HSA-eligible. You can use pre-tax dollars from your Health Savings Account to pay your monthly membership fee. This makes your membership effectively cheaper because you are paying with money that was never taxed. For 2026, HSA contribution limits are $4,300 for individuals and $8,550 for families. HSA funds roll over year to year with no use-it-or-lose-it rule. If you have an FSA, check with your administrator as eligibility varies by plan.

We recommend keeping a catastrophic or high-deductible plan for emergencies, hospital stays, and specialist referrals. Your VitaLife membership covers primary care but isn't a substitute for insurance in situations requiring hospitalization or surgery.

Yes. There are no long-term contracts. You can cancel your membership at any time. The Metabolic Reset Program is a 5-month structured commitment because it's a clinical program.

Concierge medicine typically costs $2,000-10,000 per year and still bills insurance. DPC is $150-500/month and doesn't involve insurance at all. DPC keeps costs lower by eliminating the administrative overhead of insurance billing.

Yes, we see Medicare-eligible patients. As a Direct Primary Care practice, we operate outside of Medicare billing for our membership services. Medicare patients can still use their coverage for referrals, imaging, specialist visits, labs ordered outside our practice, and hospital care. We coordinate with Medicare for everything outside the clinic. Contact us to talk through how your specific Medicare plan works with a VitaLife membership.

Your VitaLife provider identifies when specialist care is needed, gives you a referral, and stays involved in your care throughout. Your insurance covers the specialist visit. We make sure your specialist has the context they need from your labs and history so you are not starting from scratch every time you walk into a new office.

Your insurance covers hospital costs. Your VitaLife membership covers your primary care on both sides of a hospital stay. Courtney can coordinate with hospital doctors, monitor your case, and help manage your recovery and follow-up care. You will not get lost in the handoff between the hospital and outpatient care.

For true emergencies, call 911 or go to the nearest ER. Your insurance covers emergency and hospital care. What VitaLife does is make real emergencies rarer. Members have same-day access and direct provider contact, which means a lot of things that would have sent someone to urgent care get handled through a quick call or same-day visit instead.

VitaLife is a Direct Primary Care practice in Show Low, Arizona. We offer membership-based healthcare including primary care, hormone therapy, weight loss programs, peptide therapy, and functional wellness. We also offer walk-in services like IV therapy, NAD+ infusions, and aesthetics that don't require a membership.

Monday through Friday, 8:00 AM to 5:00 PM. We're closed on weekends. Members have after-hours access to their provider for urgent matters.

Yes. We start with thorough labs so your provider has real data before your first consultation. This makes your initial visit more productive and allows your provider to build a plan based on what your body actually needs.

Step 1: Book and complete a lab draw. Step 2: Your provider reviews your results. Step 3: Initial consultation where your provider discusses findings and builds your care plan. Step 4: Choose your membership and begin treatment.

Most patients experience few side effects when hormones are properly dosed and monitored. Common adjustments in the first few weeks include mild acne, water retention, or mood fluctuations as levels stabilize. Courtney monitors your labs at regular intervals and adjusts dosing based on your response. Serious side effects are rare with proper clinical oversight.

We adjust. Unlike a 10-minute visit where you get a prescription and a 6-week follow-up, VitaLife patients have direct provider access between visits. If something feels off, you message Courtney directly. Dosing changes, protocol adjustments, and alternative approaches are all part of ongoing care. We don't set it and forget it.

Telemedicine Questions

Yes. We offer telemedicine visits across the entire state of Arizona. Video consultations for primary care, hormone therapy, weight loss management, and prescription oversight.

Book a video visit through our patient portal. Labs can be drawn at a local facility near you. Your provider reviews results and conducts the consultation via video. Prescriptions are sent to your preferred pharmacy.

Yes. Hormone therapy consultations, lab reviews, and prescription management are all available via telemedicine for patients in Arizona.

No. IV therapy, NAD+ infusions, and aesthetics services require an in-person visit at our Show Low clinic.

Aesthetics Questions

No. All aesthetic services are open to everyone. We also offer an Aesthetics Membership at $75/month that includes a monthly facial, a free Cytopeel upgrade, and 10% off all aesthetic services.

Cytopeel facials (5 tiers from $175-$300), custom facials (from $85), chemical peels ($95-$180), microneedling ($275, or $525 with PRP), and add-ons like dermaplane ($30) and jelly mask ($15).
