VitaLife is a Direct Primary Care (DPC) practice located in Show Low, Arizona. We operate on a membership-based, direct-pay model. We don't bill insurance for membership services. Our services include primary care, hormone therapy, weight management, peptide therapy, IV therapy, NAD+ therapy, aesthetics, and functional wellness.

3. Membership Terms

Membership plans are billed monthly unless otherwise stated. There are no long-term contracts, and you can cancel at any time with 30 days notice. The Metabolic Reset Program is a 5-month structured clinical commitment. Membership fees are non-refundable for the current billing period. 4. Payment Terms

VitaLife is a direct-pay practice. Payment is due at the time of service or according to your membership billing cycle. We accept major credit cards, debit cards, and HSA/FSA cards where eligible. We don't bill insurance companies. We can provide superbills upon request for potential reimbursement from your insurance provider.

Services are provided by licensed healthcare providers. The information on our website is for general informational purposes and doesn't constitute medical advice.
